Valid Visa: A photocopy of a valid visa or electronic visa (e-visa) for the destination country. (Note: For countries offering Visa-on-Arrival, an entry ticket or confirmed travel itinerary must be shown).
Confirmed Air Ticket: A photocopy of the confirmed return air ticket or travel itinerary.
National Identification: A photocopy of the applicant's National ID (NID) card or Smart NID.
Form TM: A completed and signed Form TM (Travel/Miscellaneous), which is the standard central bank application form provided at the bank branch.
